Cannon EyeCare is in network with Premera, Regence, HMA, Lifewise, Uniform Medical, Blue Cross/Blue Shield and Kaiser Permanente’s Access, Options, and Omni PPO plans.
Your vision insurance carrier may be different than your medical insurance carrier. We do not accept independent vision plans such as VSP, Eyemed, Davis Vision, or Spectera due to unreasonable contractual requirements. Additionally, please note that we accept Medicare and Medicare Advantage plans. However, it’s important to mention that while we can typically bill traditional Medicare plans, we usually cannot bill a Medicare Advantage HMO plan.
In order to help you prepare for your appointment, please read the documents below. Please print & complete our patient intake form and bring it to your appointment. Otherwise please arrive 15 minutes prior to your appointment time to fill out the paperwork in office.

If you need to request a copy of your medical records to be sent to (or from) us, please complete the Medical Information Release Form and return it to the doctor’s office.

We are in network with the major local health insurance companies. Cannon EyeCare does not accept any 3rd party vision plans such as VSP, Davis, or EyeMed due to unreasonable contractual stipulations.
