Cannon EyeCare is in network with Premera, Regence, HMA, Lifewise, Uniform Medical, Blue Cross/Blue Shield and Group Health’s Access, Options, and Omni PPO plans. Your vision insurance carrier may be different than your medical insurance carrier. We do not accept independent vision plans such as VSP, Eyemed, Davis Vision, or Spectera due to unreasonable contractual requirements.

In order to help you prepare for your appointment, please read the documents below. Please print & complete our patient intake form and bring it to your appointment. Otherwise please arrive 15 minutes prior to your appointment time to fill out the paperwork in office.

If you need to request a copy of your medical records be sent to (or from) us, please complete the Medical Information Release Form and return it to the doctor’s office.
